Applicant Priority
The Patient: If the patient is 18 years or older, they should complete the application first.
Spouse or Parent: If the patient does not qualify or is a minor, a legal spouse or parent may apply on their behalf.
Responsibilities of a Secondary Applicant
If a spouse or parent applies for the patient:
Responsibility: The applicant is the primary account holder and is responsible for all payments.
Verification: The applicant must be present at the practice to show proof of ID when the transaction is processed.
