To apply for a PatientFi payment plan, applicants must meet specific age requirements. If a patient is a minor, a parent or guardian can apply on their behalf.
Age Guidelines
Standard Requirement: Applicants must be at least 18 years old.
Alabama Residents: Due to state law, applicants in Alabama must be at least 19 years old.
Applying for a Minor
If the patient is under the age of 18:
A parent or legal guardian may complete the application on the patient’s behalf.
The parent/guardian will be the primary account holder and is responsible for all payments.
The parent/guardian must be present to provide a valid photo ID at the time of the transaction.
